The MIME type is currently under review by the IESG (shepharding AD is lisa at osafoundation.org). I have now committed a patch to serve documents with this MIME type.

The application form is not sufficient as we don't want a vnd. or prs. registration. The procedure is described in http://www.rfc- editor.org/rfc/rfc4288.txt I have submitted an Internet Draft, and once that is processed, we need to inform ietf-types@iana.org, and wait at least two weeks for feedback. After integrating feedback into the document, we then need to ask the IESG to approve the draft as a Proposed Standard, at which point the IANA Considerations section of the draft will take effect and the name will be registered.
